New kernel yes, modifying something you've built once maybe depends on how
drastic. It can't hurt to do a mrproper though.

if you use it to upgrade kernel-source and kernel-headers and your running
a custom kernel you wanna rebuild.

that comes from either "make install" or "make bzlilo" (no different the a
vanilla src from kernel.org)

Yes for one leave the entry in lilo.conf, and edit the toplevel Makefile
to change the EXTRAVERSION to something we'll neve use like say "-1mine"
