Spam Assassin is worth looking at http://spamassassin.sourceforge.net
It performs textual analysis of mails and scores them. It also looks up known
spammers and includes the Vipul's Razor collective spam database that was
featured on Slashdot a couple of days ago.
The Perl version is probably a bit slow for ISP use, but they have now
introduced a daemon version with ISP's in mind.
Here is an example of the header it adds to any Spam mail it finds
