I got my SB16 to work by removing the "options sb isapnp=1" line in my /etc/modules.conf and replacing it with: options sb io=0x220 irq=9 mpu_io=0x300 dma=0 dma16=5
Otherwise, it kept trying to use irq=5 which was in use by another adapter. Of course, you want to do a cat /proc/interrupts to make sure whatever irq you are trying to assign is available.
