My email client places the cursor at the top of an email when I reply, so that's where I generally type. This is the GMail client. AFAIK many others do the same.
I don't have any trouble following emails whether they top, bottom or midpost. I don't really see how some people can get confused by this and I guess if they choose not to be helpful because someone doesn't follow the rules explicitly then it might reflect badly on the list... It certainly scares off newbies if they get a flame (no matter how minor) for not posting 'the right way'.
 
I've been on the internet nigh on 10 years and I've posted on many mailing lists in that time. Never have I seen such a big hoo-haa over where people reply. My email programs seem to quote and display replies correctly so I have no trouble reading them no matter where people post their replies.

Seems at least top posting has gone off the deep end, not following the
html stuff - but I used to come here to try to read and learn.  KInda
hard to do that even when some people seem to prefer to top post -
against mailing list Etiquette, then just when you least expect it,
they bottom post.
