Could be a bad refresh rate, check both Horizsynch and VertRefresh in XF86Config (resp. XF86Config-4 if you are running X.4.0.3). Do they fit with your monitor specs. A stupid question: do you have only one Section "Screen", Section "Device" and Section "Monitor" and if not are you sure which one is being used? A log could be useful in finding out what is the problem...
