I just got the wheel on my intellimouse working last week. Make sure that
gpm is correctly configured. I am using XFree86 4.1.0, but the mouse configuration may
be similar.

Best of luck.

<gpm.conf>
device=/dev/psaux
responsiveness=
repeat_type=
type=imps2
append=""

<XF86Config-4>
   Identifier  "Mouse1"
   Driver      "mouse"
   Option "Protocol"    "IMPS/2"
   Option "Device"      "/dev/mouse"
   Option "Buttons" "3"
   Option "ZAxisMapping" "4 5"
