Yes, gpsbabel (.org) should be able to convert this file. An appropriate command line might be: gpsbabel -i unicsv -f pathtofile.csv -o gpx -F output.gpx
gpsbabel can apparently detect what each field means intelligently, see http://www.gpsbabel.org/htmldoc-development/fmt_unicsv.html for details.
