Try gpsbabel.:

gpsbabel -i unicsv -f inputfilename.csv -o gpx -F Outputfile.gpx

This command only pass lat/lon points (waypoints) to lat/lon points in 
gpx format (waypoints). If you want to have a route, you have to run

gpsbabel -i unicsv -f inputfile.unicsv -x transform,trk=wpt -o gpx -F 
outputfile.gpx

and if you don't have timestamps in points, you should run 
bogustimestamp.py
