There's also the option of using osmarender (http://wiki.openstreetmap.org/wiki/Osmarender/Howto) with a custom rules file (osm-map-features-z17.xml on that page). This is only really feasible for individual maps because editing the XML is quite nasty, but the advantage is that you get a very fine-grained level of control. Also you can edit the created SVG with an editor like inkscape. I've used this to make a map for a conference (http://cp2010.cs.st-andrews.ac.uk/map.pdf) and it worked particularly nice for the labels, which I was able to rearrange in the SVG.
