Thanks everyone - I have this working now. I created a reverse DNS entry for my ESXi host and played with the options a little. Strangely the options didn't do what I expected. Setting root=10.0.0.52,rw=10.0.0.52 (which is my ESXi host) didn't work. Ultimately I removed those options completely and it worked (although I found that root=,rw= also worked).
