Replace cache_fds parameter and handling with better logic
We don't need a config param because we always "cache" fds...
We must always deny opening an fd if the hard limit is hit.
Improve logging when high water and hard limit are hit.
Move mdcache_lru_fds_available into mdcache_lru.c.
