I had this problem and eventually found that I had neglected to set my
$DBPath in all of Porttracker.pm, Porttracker.php and GenPortGraph.pl

I think I neglected the change in GenPortGraph.pl (It's not declared
very near the top of the file and is easy to miss).

Once I had the correct path in all locations I found the graphs were
created just fine.

Of course, other things like can apache read your RRDs also apply.
