Have you checked permissions for the directory the RRD files are located in? The nfsen user needs read/write access. Same user should be configured in the Apache setup. Also check your nfsen.conf, make sure everything is OK there. If running RRDTool 1.3, go back to a 1.2 version, seems to work better.
