We get a similar issue, see my post here:

We still haven't been able to find a smoking gun, we thought maybe
there were two different versions of our mapping out there that was
caching data differently but we cleared that and we still get the
issue.

If you figure anything out, let me know and I will do the same as we
continue to look into it. Our next step at this point is to either
start stepping through the code, or to implement caching at the
userland code layer.
