Just took my course a couple of months ago here in WI - $330, 2 full
days, they provided instruction, the course and the bikes.  The bikes
were all 250's, but a variety of makes and models, and all pieces of
junk. The speedo wasn't working on mine, and in my driving test I got
dinged for not going fast enough (because I didn't have a speedo!) -
go figure.  You had to bring you own gear.

Once I finished the course and took the written and practical test I
got a waiver to take to the DMV that got me out of taking the written
and driving test administered by the state, but because I did not have
the temps when I showed up to get my M endoresment I had to take the
written temps test.  Of course, having just taken the class and
passing the more strenuous M endorsement test, the temps was a piece
of cake.  They gave me 30 min to complete it - I think I took 5.
