I once had a deep gouge all the way around my rear tire, I had to replace it 
with only about 2,000 miles in it.  I did remember hearing a strange rubbing 
sound for about 15 second and the a clunk a day or so before I saw the damage 
to the tire.  My best guess is that something got stuck in my tire and fender 
between the tire and frame and basically rubbed away the tire until it could 
fit between the tire and fender and the flew off the bike.  Looks like 
something similar could have happened to your tire.
