Perhaps there is a misunderstanding. I've understood ["a free and open 
book"](https://archive.is/h6t5X#selection-121.131-121.150) to mean:

  * made freely available - with no restrictions, and at no cost; and
  * created and edited in the open, like a wiki or VCS.

There's nothing wrong with writing a book for profit and not giving it away for 
nothing, but then the terms "free and open book" don't apply.
