Are you perhaps storing the database on a networked filesystem (NFS,
SMB, whatever)?
Berkeley DB can't work properly with networked filesystems, due to some
inner magic of Berkeley DB.

It's not on a networked filesystem, but it is using a virtual hard drive
provided by vmware. That disk image is on a local drive.
