It's one of those crusty MH configuration options.  Here's the
description from the top-level READ-ME file:

     ISI
          When using "repl -ccme", only "cc:" the first address
          found which belongs to the user; any other Alternate-
          Mailboxes do not receive "cc:"s.

I think there might have been some other effects of [ISI], too.
But, at least, it looks like this explains the #ifdef.
