                          Why not? What you have suggested is too right. When a 
manager is unable too achieve target he is fired from job in private concern. 
If he lead to any loss due to his mistakes even he is liable to be sued. Do you 
think such accountability is applicable only in private firms? What about 
social life? Anything can happen. But those who is responsible will just sit 
and watch and give some lame excuses or order some formal inquiry just to show 
they are concerned about the incidents(that too when the case get serious such 
as death). You may know that CIA director was fired when for his failure to 
stop Indian nuclear test. Our former defence Minister Mr. Krishna Menon offered 
to resign when China occupied much land in Arunachal paradesh.

                So I strongly believe those people who should act should be 
cornered to act at any cost. No compromise should be there. Hope you understand 
me.
