Page numbers were starting in the table of contents at 1, but the table of
contents was also starting the numbering for the chapters/sections/subsections
at 1. This goes away in 1.7 as we're using ASCiiDoc so I've formatted the table
of contents using numerals for now so it doesn't conflict with the regular
numbering of the chapters/sections/subsections.
