URL: https://github.com/apache/apisix/pull/14119

   ### Description

`core.request.headers()` reads request headers with `ngx.req.get_headers()`,
which returns the first 100 headers by default. So `request-validation` checks
`header_schema` against those 100 headers, and `opa` sends only those 100 in
`request.headers`. Headers after the 100th are still forwarded to the Upstream.

This PR documents that limit in the `request-validation` and `opa` plugin
docs (English and Chinese) and points to `proxy-rewrite`'s `headers.remove` for
keeping a header from reaching the Upstream.
