[ASTERIXDB-3259][MTD] Include 'database' in DataSourceId & EntityId

Details:
Include 'database' in DataSourceId, DataSourceIndex, and EntityId.
- Change 'Feed' tuple translator to handle 'database' value and
pass it to EntityId.
