Fix convertion of view states
It is possible to have one or multiple view states to be nil when their
views do not emit documents.
As a result update and purge sequences could be incorrectly set to 0 during
init_state migration for missing or old headers.
You can merge this pull request into a Git repository by running:
$ git pull https://github.com/cloudant/couchdb-couch-mrview
