**Please provide a description of this PR:** Separate the ReAct agent prompt into shared, act-phase, and final-answer policies. The final-answer prompt now explicitly has no tools and relies only on evidence already present in the conversation. This also simplifies the shared prompt around stable diagnostic principles, treats external context as untrusted data, defines behavior for incomplete evidence, and removes redundant tool-name injection and infrastructure-specific rules. Adds prompt rendering tests covering phase-policy isolation, tool binding, final-answer tool exclusion, and percent-character preservation.
