Updated CopyOnWriteHashSet's iterator implementation to support iterator.remove(). Updated JCAManagedConnection to not use iterator.remove(). It will now collect all items to remove and then use the CopyOnWriteHashSet removeAll functionality to avoid unnecessary intermediary collection creations.
