Robert Middleton resolved LOGCXX-547.

Resolution: Fixed
Documentation and tests have been added in order to make it easy to completely
disable location information. Just define LOG4CXX_DISABLE_LOCATION_INFO and
the location info will be set to default values(a question mark)
