This PR simplifies the behaviour of the FileAppender LazyFileCreation option by delaying file truncation. The motivation in #775 to behave differently when the append option is false came from test cases that expected truncated files. The opt-in decision came after the initial implementation. The test case does not opt in to LazyFileCreation, so we can simplify the behaviour. I was surprised that the files mentioned in unused appender configurations (e.g. [multiprocess.xml](https://github.com/apache/logging-log4cxx/blob/master/src/test/resources/input/rolling/multiprocess.xml)) with LazyFileCreation set to True were truncated. -- This is an automated message from the Apache Git Service.
