Currently, cause exception is ignored, in order to better debugging, we'll
construct cause exception to super Exception.
Changes proposed in this pull request:
- Add constructor to support cause exception in PipelineSQLException super
class
- Refactor PipelineSQLException sub-classes
- Enable cause exception on toSQLException
