Optimize Codex policy and harness efficiency (#39819)
* Restrict test coverage to production sources
- allow test subjects only under src/main
- exclude src/test code from additional coverage requirements
- preserve compilation and test execution for test-only changes
- update the focused policy canary
* Optimize Codex policy and harness efficiency
- reduce instruction and skill-loading overhead
- use impact-scoped policy verification
- trim evaluator tools and harden semantic canaries

1. Perform the post-write task-delta audit required by `AGENTS.md` with the
read-only diff, surrounding context, and acceptance checklist. Confirm every
task-introduced hunk is necessary and allowlisted, prohibited paths have no
task delta, architecture changes were authorized, tests protect owned behavior,
and unattributed work remains untouched.
2. Apply every triggered coding-standard and simplification rule to the
effective task delta. Treat scripts, searches, formatters, compilation, tests,
benchmarks, and review tools as evidence rather than proof, and fix every safe
in-scope violation.
3. Run the applicable verification and non-regression checks. An invalidated,
unmatched, missing, or inconclusive required result keeps the task incomplete.
-4. Use bounded self-review only when inspection proves that the task is a
standalone local change confined to one owning module, has no public or
externally visible contract impact, and has no architecture, SPI, signature,
visibility, module-dependency, shared-owner, cross-module, protocol, dialect,
concurrency, security, performance, deletion, last-consumer, or
unresolved-evidence risk. Review root-cause and fix mapping, affected behavior,
side effects and regressions, contracts, tests, [...]
-5. Invoke `$review-pr` in Formal Review Mode when the user requests a PR,
mergeability, or formal review; the task targets or backs a PR; or any risk
excluded from bounded self-review is present. For a PR-backed candidate, apply
its public-head and remote-evidence checks; for a standalone high-risk
candidate, review only the original task baseline and task-owned delta.
+4. Use bounded self-review for an internal local candidate when inspection
proves that its frozen owners and behavior are fully understood, module
dependencies and public or externally visible contracts are unchanged,
applicable verification is complete, and no Formal Review trigger below is
present. The candidate may span files or modules when they implement one
bounded behavior and verification covers every changed owner and affected
consumer. Review root-cause and fix mapping, affecte [...]
+5. Invoke `$review-pr` in Formal Review Mode when the user requests a PR,
mergeability, or formal review; the task targets or backs a PR; or the
candidate changes a public API, SPI, externally visible contract, module
dependency, security or concurrency behavior, performance-sensitive behavior,
or contains deletion, an irreversible action, last-consumer removal, or
unresolved evidence. For a PR-backed candidate, apply its public-head and
remote-evidence checks; for a standalone triggered [...]
6. Fix every safe in-scope required finding, repeat the task-delta audit,
rerun invalidated checks, and repeat the applicable review. Stop at the
existing gate for a scope expansion, unresolved architecture choice, high-risk
action, `Review Incomplete`, or `Needs Discussion` result.
7. Hand off a bounded-review candidate only after the review finds no required
issue. Hand off a Formal Review candidate only after the latest complete report
returns `Review Result: Mergeable`; include that report when the user requested
review output or when it materially explains a high-risk decision.
8. After a code-writing task completes, provide a commit message without being
asked; this never authorizes Git mutation. Provide manual Git commands only
when the user requests them. Propose `git commit --dry-run --only` and `git
commit --only` for only task-owned files without unrelated hunks, list a
required `git add -- <new-task-paths>` first for untracked task files, and
never execute a Git write without exact current-task authorization.

# Repository Policy Maintenance
-Apply this file only when the user authorizes changes to `AGENTS.md`, a
canonical policy source listed in `.codex/harness/agents/policy-sources.toml`,
or the harness that validates those sources. `cases.toml` is the source of the
harness catalog: each case records its group, purpose, enforcement phase, and
whether the ordinary completion loop rechecks it. Run `python3
.codex/harness/agents/run.py --list-cases` to render the table.
-
-`validate` and `trace` are local deterministic modes. `semantic` and `all`
start isolated networked Codex evaluations, so run them only when non-sensitive
policy transmission is authorized and the execution environment has network
access; use command-bound platform approval when the platform requires it.
-
-1. Treat every explicit requirement, prohibition, exception, authorization
boundary, and verification step as an independent policy capability.
-2. Before editing, build an old-to-new capability ledger and classify every
capability as exactly preserved, changed with the user's explicit authorization
in the current task, or restored before handoff.
-3. Reconcile every deleted or weakened rule explicitly. A replacement is
equivalent only when it preserves the same trigger, required or forbidden
action, scope, exceptions, and verification obligation. A positive general rule
does not replace a specific negative prohibition. Ordinary Codex competence,
implication, nearby prose, a Skill, or a canary is not evidence that any
explicit rule is preserved. Partial or implicit coverage is a regression.
-4. Never remove, weaken, merge away, or broaden an exception to a capability
unless the user explicitly authorizes that exact policy change. When
equivalence is uncertain, keep the existing rule.
-5. Before editing, classify the verification impact of the exact planned
changes, capture a V0 for every semantic case and source-read trace required by
that classification, and capture a source snapshot for every manifest entry.
Reuse an existing V0 only when its manifest, bundle, cases, runner,
semantic-surface fingerprints, case set, and complete source snapshot exactly
match the current surface. Capture a full-suite V0 only when step 9 requires
the full suite. Then change one coheren [...]
-6. Count a canary as passing only when its decision matches, every required
action and reason is present, and every action outside its complete
allowed-action set is absent. A partial forbidden-action list is insufficient.
-7. Change an established canary contract only when the normative policy and
observed output prove that it rejects policy-permitted behavior. Record the
exact mismatch and preserve the required decision, action, reason, and
prohibition coverage. Do not weaken a canary or edit the runner solely to
accept one model's wording. When the user explicitly authorizes a case-contract
change, name only that case with `--authorized-contract-change`; every other
changed or removed critical contract r [...]
-8. After each coherent working-revision edit, run `--mode validate`, every
added or changed semantic case, every semantic case whose supplied policy
assertions changed, and source-read traces for every profile that contains a
changed source or whose ordered source list changed. A root `AGENTS.md` working
revision must add or change the semantic cases that directly cover every
authorized behavior change, run those cases with `--case`, and must not pass
`agents` to `--impact-source`; reser [...]
-9. Promote a working revision only after every check required by its
verification-impact classification passes. Run `--mode all` across every case
and trace only when `AGENTS.md` changes or when a change to shared semantic
execution, the semantic prompt, output schema, action catalog, reason catalog,
or grading logic can affect every case. For this classification, semantic
transport is not shared semantic execution: it consists only of input
partitioning, concurrency, Codex process invoc [...]
-10. Validate the manifest, exact-path reference graph, root byte limit, source
hashes, staged policy bundle, and source-read traces. A source-read trace
passes only when it proves that each required exact file was locally available
and read through EOF from both root and applicable nested working directories;
it does not prove semantic interpretation by a separate Codex evaluation.
Semantic canaries evaluate the root `AGENTS.md` decision surface unless a
separately authorized harness mod [...]
-11. Bind every legacy critical case whose controlling rule moves out of
`AGENTS.md` to its routing profile, manifest source, and complete normative
sentences with `policy_binding_profile` and `policy_assertions` in
`cases.toml`. The normalized binding set and digest prove that each routed
source still contains the bound rule, but they do not prove that a model
interpreted it. Reject a missing, shortened, moved, or changed established
binding even when the source and binding are edited together.
-12. After the first promoted policy-bundle candidate passes, run `--mode
validate` without a policy-bundle baseline and retain its summary as the
explicit local inventory-and-binding bootstrap. Pass that summary with
`--baseline` to every later deterministic candidate; the original semantic V0
for each evaluated case set remains the independent behavior baseline and is
not replaced by this bootstrap.
-13. Change an established binding only for an explicitly authorized equivalent
rule migration, name only that case with `--authorized-policy-binding-change`,
and preserve the old-to-new sentence audit as task evidence. Never use binding
authorization to weaken a rule, bypass a semantic canary, or change an
unrelated case.
-14. After the bootstrap, treat every source addition, removal, relocation,
metadata change, or content hash change as a policy-source change. Accept it
only when the user explicitly authorized that exact source and the run names
its source ID with `--authorized-policy-source-change`; this source
authorization does not replace the capability ledger, binding migration audit,
or semantic canaries.
-15. If the same policy failure appears twice and is not an environment failure
or a brittle canary contract, add one focused case instead of generic prose.
Stop after five promoted candidates or when no measurable improvement remains.
-16. After the canaries pass, complete the applicable simplification, internal
candidate review, finding-fix, and re-review steps in
`.codex/context/change-completion.md` before handoff.
-17. Validate the user-facing effect of changed `AGENTS.md` instructions in a
new task after merge or installation; the active task may retain instructions
loaded before the change.
+Apply this file only to an authorized change to `AGENTS.md`, a source listed
in `policy-sources.toml`, or the harness that validates those sources.
`cases.toml` owns the case catalog; use `run.py --list-cases` when its rendered
inventory is needed.
+
+1. Treat each explicit requirement, prohibition, exception, authorization
boundary, and verification step as an independent capability. Before editing,
build an old-to-new ledger and classify each capability as preserved,
authorized to change, or restored before handoff.
+2. A replacement is equivalent only when it preserves the trigger, required
and forbidden actions, scope, exceptions, and verification. Do not substitute
implication, nearby prose, ordinary Codex behavior, a Skill, or a canary for an
explicit rule, and retain the original when equivalence is uncertain.
+3. Inspect only the affected manifest entries and profiles before editing;
`--mode validate` remains responsible for validating the complete manifest and
reference graph. Capture the current source snapshot and keep one original
baseline across every candidate in the active task.
+4. Classify the change as deterministic-only, semantic policy, semantic case
or binding, shared semantic evaluation, or transport-only. For any category
containing semantic behavior, read
[semantic-verification.md](semantic-verification.md) through EOF before editing
and follow its baseline, canary, promotion, stability, and bootstrap rules.
+5. For deterministic-only changes, run `--mode validate` and applicable
source-read traces. For transport-only changes, also prove batch coverage,
limits, concurrency, failure propagation, no-retry behavior, and aggregation
with deterministic tests before the smallest live `--transport-check`.
+6. Validate root size, source hashes, staged bundle, exact-path references,
and applicable traces. A trace proves local availability and EOF reading, not
semantic interpretation.
+7. After required checks pass, complete `.codex/context/change-completion.md`.
Validate changed `AGENTS.md` behavior in a new task after merge or installation
because the active task may retain its original instructions.

+# Semantic Policy Verification
+
+Read this file through EOF before changing semantic policy content, a semantic
case or binding, or shared semantic evaluation behavior. `semantic` and `all`
start isolated networked Codex evaluations, so run them only when non-sensitive
policy transmission is authorized and network access is available.
+
+## Baseline and Case Contracts
+
+1. Before editing, classify the verification impact and capture V0 for every
required semantic case and source-read trace plus a snapshot of every manifest
source. Reuse V0 only when its manifest, bundle, cases, runner,
semantic-surface fingerprints, case set, and complete source snapshot match
exactly. Capture a full-suite V0 only when the promotion rules below require
the full suite.
+2. Count a canary as passing only when its decision matches, every required
action and reason is present, and every action outside its complete
allowed-action set is absent.
+3. Change an established canary contract only when normative policy and
observed output prove that it rejects permitted behavior. Record the mismatch
and preserve required decisions, actions, reasons, and prohibitions; never
weaken a canary or edit the runner solely to accept one model's wording.
Authorize only the named case with `--authorized-contract-change`; every other
changed or removed critical contract remains a regression.
+4. Bind a legacy critical case whose controlling rule moves out of `AGENTS.md`
to its routing profile, manifest source, and complete normative sentences. The
normalized binding proves that the source retains the rule, not that the
evaluator interpreted it.
+5. Change an established binding only for an authorized equivalent migration,
name that case with `--authorized-policy-binding-change`, and retain the
old-to-new sentence audit. Never use binding authorization to weaken a rule or
bypass a canary.
+
+## Working Revisions
+
+Run semantic harness commands with the canonical high-output wrapper and
retain the log path.
+
+1. After each coherent edit, run `--mode validate`, every added or changed
semantic case, every semantic case whose supplied assertions changed, and
source-read traces for profiles containing changed sources or changed source
order.
+2. For a root `AGENTS.md` edit, add or change the cases directly covering each
behavior change and run them with `--case`; do not pass `agents` to
`--impact-source` before promotion.
+3. For another semantic policy source, pass each exact source ID with
`--impact-source`; do not combine it with `--case` or `--profile`. Run a
semantic case only when its contract or supplied assertions changed, not merely
because its profile contains the source.
+4. If no case supplies changed non-root content through
`semantic_policy_assertions`, report that semantic interpretation is unproved
instead of running unrelated cases.
+5. A working revision cannot be handed off or used as the local bootstrap.
+
+## Promotion and Stability
+
+1. Run `--mode all` across every case and trace only when `AGENTS.md` changes
or when shared semantic execution, the semantic prompt, output schema, action
catalog, reason catalog, or grading logic can affect every case.
+2. Semantic transport consists only of input partitioning, concurrency, Codex
process invocation, result and log collection, and order-preserving
aggregation. Prove a transport-only change with deterministic tests for batch
coverage, limits, concurrency, failure propagation, no-retry behavior, and
aggregation, then use `--transport-check` with the smallest live semantic
selection needed for external integration. Transport mode never substitutes for
required semantic verification.
+3. A change limited to non-root policy sources, routing profiles, individual
contracts or bindings, deterministic validation or trace logic, baseline
comparison, result reporting, or transport does not require unrelated semantic
cases.
+4. Partition selected semantic cases under the input limit, run no more than
two batches concurrently, evaluate each case exactly once, and never retry an
unchanged failed batch.
+5. Use `--semantic-stability-check` only with an original V0 from the same
evaluator model and reasoning effort, the same case IDs, and complete
single-pass failure metadata. It passes only when transport is exact,
deterministic changes are authorized, the single-pass pass count is not lower,
and no more-permissive decision, forbidden action, or unauthorized high-risk
action is introduced. Wording and redundant action mismatches remain reported
but are not independently blocking when thi [...]
+6. Compare pass rate, duration, input tokens, and uncached input tokens only
for identical case sets. Treat deterministic, transport, aggregate, or safety
regressions as blocking and inspect bounded logs before another edit.
+7. Only a candidate passing every check required by its impact classification
may enter bootstrap, completion review, or handoff.
+
+## Source Integrity and Bootstrap
+
+1. Validate the complete manifest, exact-path reference graph, root byte
limit, source hashes, staged bundle, and applicable source-read traces. A trace
proves exact local availability and EOF reading from root and nested
directories, not semantic interpretation.
+2. Semantic canaries evaluate the root `AGENTS.md` decision surface unless an
authorized harness path supplies synthetic, non-sensitive policy assertions.
+3. After the first promoted bundle passes, run `--mode validate` without a
policy-bundle baseline and retain its summary as the local
inventory-and-binding bootstrap. Use it as the deterministic baseline for later
candidates while retaining the original semantic V0 for each evaluated case set.
+4. After bootstrap, authorize each source addition, removal, relocation,
metadata change, or content-hash change by exact source ID with
`--authorized-policy-source-change`. This does not replace the capability
ledger, binding audit, or semantic canaries.
+5. If the same policy failure appears twice and is not environmental or a
brittle canary, add one focused case instead of generic prose. Stop after five
promoted candidates or when no measurable improvement remains.

# ShardingSphere Codex Development Guide

+Follow this guide; paths are repository-relative.
## Instruction Sources and Routing
-1. `CODE_OF_CONDUCT.md` governs contribution, Java, and unit-test style.
Before changing code or tests, inspect the applicable section and record
controlling decisions.
-2. Before Maven, E2E, Proxy startup, database clients, IDE/MCP run
configurations, large structured analysis, or a command likely to exceed 100
output lines, read or reuse `.codex/context/token-efficiency.md` and follow its
Mandatory Execution Contract.
-3. Before the first code-affecting write, read
`.codex/skills/code-implementation/SKILL.md` through EOF. It covers code,
tests, scripts, build logic, generated source, and behavior configuration;
read-only and prose-only work does not activate it. Its implementation
reference is required, as is every in-scope candidate violation. A Skill grants
no authority or scope.
-4. Skill discovery is diagnostic. If it omits `code-implementation`, load that
repository file directly. A missing required source keeps ordinary writes
read-only; only an exact user-authorized policy or harness repair may use
`.codex/harness/agents/policy-maintenance.md`, never for production or tests.

`.codex/skills/code-implementation/references/rules/implementation.md` through
EOF without activating the write workflow. Also read `testing.md` when deciding
test requirements or analyzing tests, `artifact-removal-and-contract-impact.md`
when its gate below applies, and `non-regression.md` when reviewing a
current-task candidate or a credible functional or performance risk.
-6. Before classifying an artifact as unused or removable, read that removal
reference through EOF and apply its complete evidence gate. Use single-model
convergence only when the task removes the last production consumer and proves
no compatibility contract remains. This route is read-only.
-7. For runtime diagnosis, read `.codex/context/runtime-triage.md` through EOF.
Diagnosis remains read-only; if the user authorizes a fix, activate
`code-implementation` before the first code-affecting write.

`code-implementation`.

`.codex/context/cross-cutting-skills.md` through EOF and apply its triggers and
limits.
-9. Before choosing, running, or assessing repository verification in any task,
read `.codex/skills/code-implementation/references/verification.md` through
EOF. For an authorized change, build, implementation, or fix, read
`.codex/context/change-completion.md` before its completion loop or handoff.
-10. Before creating or revising documentation, Skills, prompts, comments,
configuration descriptions, or other prose, read
`.codex/context/documentation-wording.md` through EOF and apply it.

outside this file.

### Changing Repository Policy

integrity, candidate, canary, performance, and repair requirements; reject
critical regressions and oversized root guides.

## Authority and Safety
-- Answer, explain, review, diagnose, audit, and plan requests are read-only:
inspect and report without editing the reviewed target.

cleanup, and system changes require current-task authorization. Naming the
exact non-code target authorizes it unless another gate applies.

non-code or system write, Git or remote mutation, remote transmission,
destructive action, and file-changing tool. Consolidate proven needs by exact

### Git Is Read-Only by Default
-The user owns every Git state change. Read-only `status`, `diff`, `show`,
`log`, `blame`, `grep`, and `ls-files` are allowed. Resolve a Git write
read-only and run it only with current authorization for its exact operation
and target; do not extend authority.
-
-Without exact authority, do not change the index, working tree, refs, remotes,
branches, tags, stashes, worktrees, or submodules. This includes `add`,
`commit`, `push`, `fetch`, `pull`, `merge`, `rebase`, `reset`, `restore`,
`checkout`, `switch`, `clean`, `cherry-pick`, `revert`, branch, tag, stash,
worktree, and `submodule update`; do not stage or use Git to roll back.

files.
### Remote Writes and Sensitive Data
-- A local coding request does not authorize updating an issue, PR, review,
repository, deployment, production API, connector, cloud task, message, or
other remote state. Write remotely only when the current request authorizes the
exact action and target.
-- For GitHub, prefer `GH_TOKEN`, then `GITHUB_TOKEN`, without exposing either;
call the API directly when available, otherwise use `gh`.
-- Do not send credentials, tokens, private keys, private logs, proprietary
source, personal data, connection strings, or other sensitive repository data
externally. Redact sensitive values from commands, summaries, and retained
artifacts.
-- Do not invoke another Codex task or external review service. Only the policy
harness may run an isolated task, with synthetic non-sensitive cases in a
read-only ephemeral environment and no source, logs, task data, or sensitive
values. Otherwise use bounded self-review in the active task.
+- A local request does not authorize a remote update; write only when the

### Destructive and High-Risk Local Actions
-Before deleting files or data, bulk-editing non-code artifacts, removing
Docker containers, changing global configuration, permissions, or packages, or
taking another destructive local action:
-
-1. Resolve and inspect the exact targets read-only.
-2. State the impact and whether recovery is reliable.
-3. Confirm unless the user already authorized the exact targets and a reliable
recovery path exists.
-
-A reliable recovery path is a verified backup or source with restore steps, or
a deterministic rebuild or re-download from a confirmed origin. Git requires
separate authority for the exact restore. If recovery is unverified, state that
no reliable rollback exists and confirm again.

-For Docker cleanup, distinguish reproducible images from containers, volumes,
and local data. In-scope images proven unused and reproducible need no further
confirmation; containers, volumes, local data, and uncertain images remain
unauthorized.

of reliable rollback, and potential consequence.
+For Docker cleanup, distinguish reproducible images from containers, volumes,

confirmation; the other categories remain unauthorized unless named.
## Evidence, Scope, and Planning
-Evaluate user premises affecting correctness, scope, compatibility, safety, or
cost. Separate evidence from inference, assumption, and preference. If evidence
contradicts or cannot support an action, report its impact and the smallest
alternative or decision before acting; do not add generic caveats or expand
scope.

cost. Separate evidence from inference and preference; when evidence

-Before editing, restate the goal, non-goals, forbidden tools, and output
constraints. Inspect affected ownership, code, tests, contracts, configuration,
registrations, boundaries, precedent, and instructions. Identify reuse,
compatibility, prohibited paths, and verification, then record a compact
acceptance checklist and 3–10 steps for non-trivial work.

acceptance checklist and a 3–10 step plan for non-trivial work.
### Strict Scope and Task-Delta Gate
-1. An active task is one independent, verifiable objective, including its
corrections and follow-ups. Start a new baseline only after completion and an
explicit independent objective; if ambiguous, remain read-only and confirm.
Findings, failures, reviews, or module mentions do not start a task.
-2. Derive acceptance criteria only from requested outcomes, proven direct
prerequisites, and focused regression protection. Cleanup, refactoring,
generalization, consistency, adjacent fixes, and optional improvements are not
requirements. A prerequisite qualifies only when omitting it prevents requested
behavior, compilation, or scoped verification and no smaller in-boundary option
exists.
-3. Before a file-changing tool, record original status and relevant diffs,
infer the smallest owning paths, and freeze `file -> allowed intent -> unmet
criterion`. A user-named module or path is the maximum boundary; an allowed
file does not authorize unrelated hunks.
-4. Keep that baseline and boundary across follow-ups, reviews, failures, and
verification. Expand only after exact additional path and intent authorization;
append without rebaselining and keep other scope read-only.
-5. Map every task hunk to an unmet criterion and inspect it immediately.
Before an outside-boundary edit, report its evidence, minimum scope, affected
contracts, and consequence of declining it.
-6. Preserve all pre-existing and unattributed changes. Do not overwrite,
format, remove, roll back, or claim them; absence from the baseline does not
prove task ownership.
-7. After the last write, audit the delta against the original baseline. Remove
only unnecessary task changes that are safely separable, then hand off `file ->
changed behavior -> acceptance criterion -> necessity`.

### Architecture Change Gate
-Architecture changes include public contracts, SPIs, extension or loading
contracts, `final`, visibility, inheritance, constructors, signatures, module
dependencies, and shared ownership. Before editing, report ownership and
behavior, reuse or delegation, compatibility, and minimum files and tests. Make
only the requested change; confirm materially different unresolved choices and
always gate an SPI change.

dependencies, and shared ownership. Before editing, report ownership, reuse or

change; confirm materially different choices and always gate an SPI change.
If the user rejects a design, stop patching it. Remove only its proven task
changes, preserve unrelated work, and redesign from the last confirmed
boundary. Use precise edits unless the task authorizes an exact Git restore.
## Task Code Size Limit
-Do not add over 10,000 physical production and test source lines in one active
task without explicit authorization. Estimate before writing and measure after
each write against the original baseline; all tool, script, Skill, module, and
later-turn output counts.
-
-If the total would exceed the limit, stop, report current and projected totals
and paths, and propose the smallest independently verifiable decomposition
without expanding scope.

## Specialized Workflows
-Use matching Skills:
-
-- Issue diagnosis and maintainer replies: `$analyze-issue`.
-- Unit-test generation or coverage: `$gen-ut` with `code-implementation` and
its pre-write non-regression assessment.
-- PR correctness, mergeability, CI, or pre-handoff review: `$review-pr`;
Discussion Reply only when requested. Complete reviews or recommendations use
Formal Review with `### Result`, one `Review Result`, `### Evidence`, and `###
Coverage`; Coverage identifies local-only changes. Use `Review Incomplete` only
when an outcome-sensitive decisive fact is proven unavailable. Standalone
review is read-only and skips `code-implementation`.

or create one for the task.

expand authority or scope.

`.codex/context/change-completion.md`, except a governed standalone restoration
or rollback. Use its bounded self-review only for a proven low-risk standalone
change; use `$review-pr` Formal Review for every trigger listed there. Fix safe
in-scope findings and hand off only after the required delta audit, checks, and
review pass.
