Same issue here. It lists the tables fine, but when trying to select a table this error occurs. I noticed that its putting /undefined/false on the end of the request url. when removing it, it seems to load fine. werkzeug.exceptions.NotFound: 404 Not Found: The requested URL was not found on the server. If you entered the URL manually please check your spelling and try again.
