Still being reported in July 2025 with Helm deployments using WebSockets. Common culprits are Gunicorn worker exhaustion or misconfigured WebSocket/async query settings. Ensure your deployment has sufficient Gunicorn workers and that `GLOBAL_ASYNC_QUERIES` is properly configured.
