Review Comment:
The per-call transport has the same cancellation-token precedence issue:
`_responseReadTimeout` is initialized by `FlushAsync`, so this expression
ignores the token supplied to `ReadAsync`. A caller cannot cancel a blocked
response read unless the flush token was also canceled; link the read token
with the timeout source instead of choosing one.

Review Comment:
Using `_responseReadTimeout.Token` unconditionally discards the token
supplied to this `ReadAsync` call once a response has been received. For
example, after `FlushAsync(CancellationToken.None)`, canceling a later
`ReadAsync` token will leave a blocked body read running until
`_connectTimeout`; before this change the passed token was used directly.
Combine the per-read token with the timeout token before calling the stream.
