Well ummm I just put mcafee's commandline product on the server , and ummm I
only paid one server licence (under $100). Not quite sure on the legalities
of it (in my defence I now a site licence for mcafee, but it would be
interesting to check the mcafee licence for their command line scanner)
mailsweeper cost is based on people protected , you can have as many servers
as you like.

A third party in NZ has release an addon (called "ReleaseIT") which enables
specific people to release email that has been quarantined via mimesweeper
based on attachment or subject or from/to. I have a PDF on it if anyone is
interested (I have no interest in the company), the product just looks
exactly what I am looking for, empowering users and making them accountable.
Secret sweeper is also a useful addon for encrypting mail between sites.

We are using MailSweeper from Content Technologies. Does all that and much
more. We not only do what you are asking for, but you can also scan for
words and phrases weighted so that a single instance will not block a mail
unless you want it to. We use it to block SPAM, Chain Letters and hoaxes as
well.
There is even a PORNSweeper component that allows you to search for too much
skin tone in a picture and filter for it. Not overly accurate, but still a
good start for keeping Porn out of your mail.
Downside is that you have to purchase a separate virus scanner that plugs
into MailSweeper. By far the most comprehensive package I have seen and
seems to be very reliable.
Tech support is a mixed bag. They do answer fast and try hard. They even
sent someone out to our site to look at our problem. But, we spent over a
year with a "crippled" install because we were told that we were the only
ones with a particular problem and then after they acknowledged the problem
it took another six months before they fixed it. On the plus side we did
use the product free for a year, even though we could only scan incoming
mail.
