We only filter at the perimeter as you are suggesting. The downside is if anything does get in you are defenseless.
Do you allow your users to outside e-mail sources such as AOL and such? We don't and have not seen a virus inside since the lovebug. If you do allow access I would recommend a scanner on each server. We use MimeSweeper along with Sophos scanning SMTP as it comes in. We have been highly successful and Mimesweeper probably has the best feature set you will find.
