There are several methods,

(1) simple backgrounds:

% output=pdftex

\starttext

\setupcolors
   [state=start]

\setupbackgrounds
   [text][text]
   [frame=on,
    rightframe=off,
    topframe=off,
    bottomframe=off,
    framecolor=red,
    rulethickness=1mm,
    frameoffset=.5cm]

(2) metapost backgrounds

\setupbackgrounds
   [page]
   [background=DemoFrame]

\defineoverlay[DemoFrame][\reuseMPgraphic{DemoFrame}]

\startreusableMPgraphic {DemoFrame}
   StartPage ;
   Field[Text][Text] := Field[Text][Text] enlarged .5cm ;
   draw (llcorner Field[Text][Text] -- ulcorner Field[Text][Text])
     withcolor green withpen pencircle scaled .5cm ;
   StopPage ;
\stopreusableMPgraphic

Method 2 can also use the values from setup commands, see metafun manual.
