Did you turn off the option 'Fit to Page' in the print dialog box of the 
Acrobat Reader? Did you really switch to the a4 papersize in the PageSetup 
Dialog Box? I need to do both. The results are very accurate (tested with 
the testpage.tex LaTeX file, less than 1mm difference).
