The problem were \index entries after a \blank (or otherwise unanchored).
When these appeared at the end of a section, the bookmark of the
following section got a {0} for each \index.
(Unanchored index entries are always wrong, but usually didn’t cause
problems so far, and still don’t if interaction is off.)
