With \framed it works, but with \externalfigure it's messed up:

\starttext
        \startplacefigure [title=90°]
                \externalfigure [cow] [orientation=90, frame=on]
        \stopplacefigure

        \startplacefigure [title=89°]
                \externalfigure [cow] [orientation=89, frame=on]
        \stopplacefigure
\stoptext
