It seems that your command \cmd places again the content… Why do you want that 
command?
This works fine:

\usemodule [annotation]
\defineannotation
 [myannotation]

\starttext
 \startmyannotation
   foo
   
   \input knuth.tex
 \stopmyannotation
\stoptext
