You can create a new float environment which inherits all values (including the 
counter) from the figure environment.

\setupexternalfigures[location=default]

\definefloat[nofigure][nofigures][figure]

\starttext

\placelist[figure]

\blank[4*line]

\startplacefigure[title=A duth cow]
        \externalfigure[cow][width=6cm]
\stopplacefigure

\startplacenofigure[title=A duth mill]
        \externalfigure[mill][width=6cm]
\stopplacenofigure

\stoptext
