Hi Jano,

this shows the right hyphenation:

    \setuplanguage[cs][righthyphenmin=2]
    \mainlanguage[cs]
    \setuphyphenation[method=traditional]
    \registerhyphenationexception[cs][cíl-em]%
    \starttext
    \startTEXpage[offset=1ex]
    \hyphenatedword{cílem}
    \stopTEXpage
    \stoptext

The value for righthyphenmin in Czech is 3. Unless you set it up to 2,
it won’t work.

I hope it helps,

Pablo
