Lots of places still don't require area codes. The NANP allows for 7, 10, or 11 digit dialing. Only a small number of large metropolitan areas are required to use 10 or 11 digit dialing. (Although with some carriers it is optional, and some added the requirements for all their locations not just the required metropolitan areas.)
Overall, it's a mess. This is the major reason configuring Enterprise Voice in Lync is so painful.
