Users with admin privs, followed by not patching on a timely schedule (and auditing your patch operation to find machines where the patches didn't install!). Seriously, those are the biggest threats. It's like trying to put the smoke back in a log after you burn it. It's much easier to keep stuff off the machines in the first place then trying to detect infections. Detection is a loosing proposition - the bad guys can mutate faster then the scan engines can keep up.
