Aye, the question is quite confusing in light of the subject matter.
Seems like one would just want to make "start.html" the top-priority
default document in the virtual directory where the app lives.

One might even go a step further and create a redirection rule or
default document at the root of the website to send people to the app.

I am not a fan of web apps that tinker with applicationHost.config
without a compelling reason...

For HTTP to HTTPS redirect, I usually just require SSL on the app and
customize the appropriate 401.x response document.
