Seems you have eliminated everything but the card and the cable. Presumably the device with good throughput is connected w/ a different cable? Have you swapped the cables? Checked to see if a desk leg is sitting one? If it is very old and brittle or if it has been crimped at some point. Had someone once make everything neat and tidy by stapling the cables in place. Replaced a lot of cables....

Speedtest.net says he's getting in excess of 20Mbps down and 5Mbps up via a wireless connection, but with a wired connection to the router his reported speed drops to 5 down/1 up, and the difference is readily apparent when browsing. Connecting with a wired connection from another machine, however, doesn't not report a slower speed and closely matches the wireless speed. He's updated the drivers for the NIC, adjusted the speed and duplex settings, disabled the software firewall, tried other ports on the router, swapped cables, but cannot improved his throughput when using an Ethernet connection from this machine.
