I did a bunch of research because I wanted to use VOIP in my new business. Ultimately, I decided to go hosted VOIP instead of taking it in-house. We wound up with RingCentral, and do not regret the choice. If you go in-house, VOIP has a series of vulnerabilities, and you should run something like VOICE FIREWALL from SecureLogix. Just reading their spec sheet PDFs will show you where the holes are that need to be covered.
