What exactly is the content of the PDF? Is it primarily text, or is it
a single embedded TIFF image, or mixed text/images?

Be aware SMTP is a text-only protocol. This means that if it's all or
mostly binary data (images, mostly) that the transmitted size can be
significantly larger than the size of the message on disk, because the
binary data is encoded into a text equivalent.
