That's #1 in my book.

Also, in rough order:

2) Not keeping your software patches up to date

3) Not filtering users web browsing

4) Not whitelisting applications (AV is a poor second choice, IMHO)

5) Not encrypting laptop hard drives

6) Buying into BYOD without good MDM (actually, buying into BYOD at
all, because, IMHO, only company-owned devices should have access to
the company network - see #1 and #4 above)
