Have a look at the FiberLogic OptiqRoute - the standard model, or the
MeshVPN if you want to aggregate VPN bandwidth.   They are resold in the U.S.
by Alvaco.com - give them a call and talk with Manny and tell him you were
referred by All Covered and I'm sure he'll make you a good deal.  The base
unit (non-VPN) is only about $1500 for two ports.

They do both inbound and outbound load balancing, and yes, through
manipulation of NAT and DNS to accomplish those ends.  Configured properly,
though, and with an understanding of the limitations and implications of the
unit, they operate flawlessly.    Both inbound and outbound load balancing
and failover are transparent and robust across diverse connections with no
need to rely on BGP or the ISP to cooperate.  It allows you to have carrier
diversity at a much lower cost and complexity level, as well as transparent
DNS-based inbound load balancing (i.e., no establishing 'www', 'www2' and
'www3' for your inbound DNS names, and so on).

I've installed a half-dozen of them over past year for small/medium business
clients, and have been very happy with the results.  One such installation
is at an ambulance company that requires 24/7 access to web-based dispatch
and timesheet systems; the OptiqRoute has worked flawlessly for them.
